Emergence of Avian Influenza A(H7N9) Virus Causing Severe Human Illness － China, February-April 2013

ヒトに重大な疾患をもたらす鳥インフルエンザA(H7N9)ウイルスの発生－中国、2013年2～4月

2013年3月29日、Chinese Center for Disease Control and Preventionは、ヒトでの感染報告のない鳥インフルエンザA(H7N9)ウイルス感染により3名が感染したことを確認し、31日、WHOに報告した。感染した3例は上海在住の成人2名と安徽省の成人1名であり、全例が重度の肺炎から急性呼吸窮迫症候群(ARDS)を来し死亡した。ヒトからヒトへの感染は認められておらず、アメリカを含む中国以外の国々では感染の報告はない。その後、4月29日の時点では126例での感染が確認され、うち24例(19%)が死亡している。発症は中国東部の安徽省(4)、福建省(2)、河南省(4)、湖南省(1)、江蘇省(27)、江西省(5)、山東省(2)、浙江省(46)、2都市(北京:1、上海:33)および台湾(1)にて認められている。感染源は調査中であるが、確診例82例中63例(77%)は動物(鶏:76%、アヒル:20%)との接触が報告され、少なくとも3名の家族症例にて限定的なヒトからヒトへの感染が報告されている。確診例の年齢は中央値:61歳(四分位範囲:48～74歳)、17例(21%)が75歳以上の高齢者であり、男性が58例(71%)を占め、小児は4例であった。4月17日時点にて82例中81例が入院、17例(21%)がARDSまたは多臓器不全により死亡、60例(74%)が入院中であり、退院したのは4例のみであった。82例と接触のあった1,689名を対象とした追跡調査では感染は確認されておらず、アメリカでは10日以内に中国から帰国した旅行者(18州にて37名)を対象に調査されたが、感染は確認されていない。中国の研究者によるゲノム解析の結果、遺伝子はすべて鳥由来であり、ユーラシア株(H7N3、H7N9、H9N2)に類似していた。CDCは入手したウイルス(A/Anhui/1/2013、A/Shanghai/1/2013)を現在解析中であり、4月22日に新しいH7検査薬がFDAの診断検査基準を満たし、使用が開始された。また、4月26日時点における中国農業部による鳥および環境検体(n=68,060)の検査では、46検体(0.07%)がH7N9陽性であった。
