Interim Adjusted Estimates of Seasonal Influenza Vaccine Effectiveness － United States, February 2013

季節性インフルエンザワクチンの有効性に関する中間補正評価－アメリカ、2013年2月

2012年12月3日～2013年1月19日にU.S. Influenza Vaccine Effectiveness(Flu VE)ネットワークに登録された咳を伴う急性呼吸器疾患により医療機関を受診した2,697例(男1,582女1,115)を対象に、A型およびB型インフルエンザウイルス感染症に対するワクチンの有効性(VE)を分析した。ここでは5ヵ所 [シアトル(ワシントン州)、マーシュフィールド(ウィスコンシン州)、アナーバーおよびデトロイト(ミシガン州)、ピッツバーグ(ペンシルバニア州)、テンプル(テキサス州)] のネットワークデータを使用した。2,697例のうちインフルエンザ陽性例(rRT-PCR法)は1,115例(41%)であり、うちワクチン接種例は367例(33%)、インフルエンザ陰性例(1,582例)では793例(50%)であり、全体での接種率は1,160/2,697例(43%)であった。男女別では男性:435/1,092例(40%)、女性:725/1,605例(45%)、年齢別では65歳以上の高齢者にて高かった(205/290例、71%)。インフルエンザAおよびB型ウイルスに対するVE(年齢、地域、人種/民族、健康状態および発症からネットワーク登録までの日数により補正)は全体で56%と算出され、A(H3N2)ウイルス感染例(n=546)におけるVEは47%(95%CI=35-58%)であり、年齢別では6ヵ月～17歳:58%、18～49歳:46%、50～64歳:50%、65歳以上:9%と65歳以上の高齢者にて有意に低値であった。B型ウイルス感染例(n=366)におけるVEは67%(95%CI=51-78%)であり、年齢による有意な相違は認めなかった。以上、インフルエンザA(H3N2)およびインフルエンザBに対するワクチンの有効性は65歳以上の高齢者にてA(H3N2)に対して低く、高齢者ではワクチン接種の有無にかかわらず抗ウイルス薬の投与が推奨される。
