乳児髄膜炎菌ワクチン接種:Advisory Committee on Immunization Practices (ACIP)勧告および理論的根拠

Advisory Committee on Immunization Practices (ACIP)は2012年10月の会議において、髄膜炎菌性疾患のリスクの高い生後6週から18ヵ月齢の乳幼児の髄膜炎菌血清型CおよびYに対する予防接種に関する勧告について審議を行った。ここではその内容と理論的根拠について報告する。2012年6月14日、髄膜炎菌血清型C、Yおよびヘモフィルスb破傷風トキソイド結合型ワクチン(Hib-MenCY-TT)が生後6週から18ヵ月の乳幼児を対象にFDAに認可された。Hib-MenCY-TTが認可される前は生後9～23ヵ月齢の乳幼児に対しMenACWY-D(2回接種)、2～55歳に対しMenACWY-DおよびMenAWY-CRM(1回接種)の接種がハイリスク例に対し推奨されており、生後2～8ヵ月齢の乳児に対するワクチンはなかった。ACIPはHib-MenCY-TTの第II相および第III相臨床試験のデータより、疾患疫学、接種スケジュール、安全性および免疫原性について検討し、すでに認可されているHib-TT、HibPRP-OMPと安全性および免疫原性が同等であること、DTap-HepB-IPV、PCV7(生後2、4、6ヵ月齢)、MMR、水痘、PCV7(生後12～15ヵ月齢)との同時接種による免疫原性の低下が認められなかったことから、Hibおよび髄膜炎菌血清型CおよびYに対する有効性を確認した。また、0～59ヵ月齢児における髄膜炎菌性疾患の発症率を分析し、費用対効果、定期予防接種プログラムへの影響、小児科医および家庭医の意見を含めて検討した結果、髄膜炎菌性疾患リスクの高い乳児、髄膜炎菌性血清型CおよびYによるアウトブレイクが発生している地域の生後6週間～18ヵ月齢の乳児への4回接種、他のHibワクチンを接種した場合は生後9～23ヵ月にて2回接種を推奨することを決定した。なお、髄膜炎菌性疾患リスクを認めない乳児に対する接種は推奨されていない。
