Early Warning Disease Surveillance After a Flood Emergency － Pakistan, 2010

洪水後の早期警告疾患サーベイランス－パキスタン

パキスタンでは2010年7～8月にかけて発生した洪水により、約1,800万人が被害を受けた。これに対し、パキスタン保健省とWHOは疾患早期警戒システム(DEWS)を構築し、アウトブレイクの検出と対応を行った。この洪水による死者は1,700人以上、損傷や破壊した家屋は1,900万棟に及び、1,000万人以上が避難できずに取り残された。2010年7月29日より1日の病気の発生数が報告され、開始から6週間において、毎週平均958～1,948ヵ所より報告があり、9月中旬までに洪水の被害を受けた120地域のうち81地域(67.5%)をカバーした。9月15日までに診察を受けた患者は計5,618,902例、うち2,174,368例(38.7%)は報告義務のある疾患であり、急性呼吸器感染症:850,292例(15.1%)、急性水様性下痢(AWD):745,532例(13.3%)、原因不明の発熱:327,453例(5.8%)を多く認め、他に皮膚疾患、イヌや蛇による咬傷、眼や耳の感染症、外傷および心臓発作等を3,444,534例(61.3%)に認めた。この期間に出されたアウトブレイク警告は130件[AWD:115(88.5%)、麻疹疑似:7(5.4%)、急性弛緩性麻痺/髄膜炎疑似:各2(1.5%)、血性下痢/マラリア擬似/急性出血性発熱症候群/その他:各1(0.8%)]であり、うち82件(71.3%)で1つ以上の微生物学的検体が提出され、55件(67.1%)がコレラ菌陽性を示した。麻疹擬似、急性弛緩性麻痺および髄膜炎擬似症例はいずれも麻疹、ポリオ、細菌性髄膜炎と確診されなかった。このDEWSにて指摘された疾患の分類、報告義務を課した疾患の不備および不適切性、地域によるデータの格差やモニタリングの困難性などの問題点が改善され、その後、2011年の台風による洪水の際には45,510,570例が診察を受け、5,752件の疾患アウトブレイク警告が出された。
