Vital Signs: HIV Infection, Testing, and Risk Behaviors Among Youths － United States

バイタルサイン:若者におけるHIV感染症、検査および危険行為－アメリカ

CDCはNational HIV Surveillance Systemのデータを用いて、2009年の若者(13～24歳)におけるHIV感染症の有病率および2010年の新規感染数を推定した。その結果、2009年末にて、HIV感染症と診断された若者は10万人あたり69.5であり、州別では2.3～562.8、西部および中西部に比べ、南部および北東部にて高かった。2010年における新規HIV感染は47,500例と推定され、うち若者は12,200例(25.7%、男性:10,100、女性:2,100)であり、人種別では黒人/アフリカ系:7,000(男5,600女1,400)、ヒスパニック/ラテン系:2,390(男2,100女290)、白人:2,380(男2,100女280)であった。感染原因は男性間性行為(MSM):72.1%、異性間性行為:19.8%、静注ドラッグ:4.0%、MSM+静注ドラッグ:3.7%であり、男性ではMSM (87.1%)、女性では異性間性行為(85.7%)が最も多く、MSMによる新規感染例8,800例のうち黒人/アフリカ系が4,800例(54.4%)を占め、ヒスパニック/ラテン系:1,900(21.6%)、白人:1,800(20.5%)であった。また、HIV感染の危険因子およびHIV検査についてYouth Risk Behavior Surveillance System(9～12年生対象、2009および2011年)およびNational Health Interview Survey(18～24歳対象、2010年)のデータを分析した結果、性行為の経験がある高校生のうち、MSMは4名以上との性行為および静注ドラックの使用率が高く、AIDSおよびHIV感染症に関する教育を受ける割合が低かった。また、性行為前に飲酒またはドラッグを使用する割合が高く、避妊具を使用する割合は低かった。2011年におけるHIV検査率は9～12年生にて12.9%であり、性行為経験者では22.2%(男17.6%、女27.2%、黒人/アフリカ系:32.0%、ヒスパニック/ラテン系:20.1%、白人:19.6%)であった。また、2010年の18～24歳における検査率は34.5%であり、女性(45.0%)より男性(24.1%)、黒人/アフリカ系(53.3%)より白人(29.8%)やヒスパニック/ラテン系(36.2%)で低かった。
