Newborn Screening for Critical Congenital Heart Disease: Potential Roles of Birth Defects Surveillance Programs － United States, 2010-2011 　

重篤な先天性心疾患の新生児スクリーニング:先天異常サーベイランスプログラムの役割－アメリカ、2010～2011年

2011年9月、米国Department of Health and Human Services(HHS)の長官は全ての新生児に対しパルスオキシメーターを用いた血中酸素濃度の非侵襲的検査により重篤な先天性心疾患(CCHD)のスクリーニングを実施するという2010年Secretary’s Advisory Committee on Heritable Disorders in Newborns and Children(SACHDNC)勧告を承認した。CDCは、州における先天異常サーベイランスプログラムの役割や能力を評価する調査と新生児CCHDスクリーニング活動を支援する準備を行うためNational Birth Defects Prevention Network(NBDPN)と提携した。この報告は、初回SACHDNC勧告発表後の2010年11月とHHS長官による承認後の2011年11月に行った先天異常サーベイランスプログラムに関する調査結果をまとめたものである。全米50州およびワシントンDCのうち先天異常サーベイランスプログラムを保有している州は、2回の調査とも43州であった。CCHDスクリーニングに関係したパイロットプログラムを保有している州の数は2010年の1州から2011年には10州に増加した。また2011年には13州のプログラムが全ての真陽性および偽陽性結果を評価するための能力を有しており、13州のプログラムは偽陰性結果を評価する能力も有していた。新生児CCHDスクリーニングに関する障壁として、スタッフや資金、法律、情報基盤の欠如などが挙げられた。プログラムの67%は先天異常の完全なデータの収集に平均12ヶ月以上要していた。CCHDスクリーニングプログラムを確実に成功させるため、各州はスクリーニング実施前に情報基盤や資源のニーズを評価するべきである。
