2012年、野生型ポリオ(WPV)の伝播はアフガニスタン、パキスタンおよびナイジェリアにて継続している。ここでは2011年1月～2012年8月におけるアフガニスタンおよびパキスタンのポリオ根絶に向けた活動について報告する。乳児の経口ポリオワクチン3回接種率(OPV3)はアフガニスタン:66%、パキスタン:75%と報告されており、両国ともに地域により異なった。この期間、59ヵ月齢以下の小児を対象とした大規模追加接種活動(SIAs)は2価(1型、2型)および3価ワクチンにより、前年、情勢不安定で近付けなかった地域を中心に実施された。アフガニスタンでは東部、南東部、南部および西部ファラー郡にてnational immunization days(NIDs):6回、subnational immunization days (SNIDs):10回が実施され、10歳以下を対象に麻疹とOPVの併用接種も行われた。パキスタンでは連邦直轄部族地域(FATA)、カイバルパクトゥンクワ州(KP)、シンド州、バローチスターン州およびパンジャーブ州南部にてNIDs:7 回、SNIDs:8回実施された。急性弛緩性麻痺(AFP)サーベイランスでは、非ポリオAFP発症数は15歳未満の小児10万人あたりアフガニスタンにて10.5、パキスタンにて7.2であり、便検体の回収率はアフガニスタンにて92%、パキスタンにて88%であった。WPV症例はアフガニスタンでは2010年:25例(WPV1:17、WPV3:8)、2011年:80例(WPV1)、2012年1～8月:17例(WPV1)であり、2010年4月以降WPV3症例は報告されていない。36ヶ月齢未満の小児は72例(74%)報告され、予防接種は24例が未接種、23 例が1～3回、25例は4回以上の接種を受けていた。発症地域は329地域のうち40地域(12%)、南西部のカンダハール、ヘルマンド、ウルスガン、ファラー郡がハイリスク地域であった。パキスタンでは2010年:144例(WPV1:120、WPV3:24)、2011年:198例(WPV1:196、WPV3:2)、2012年1～8月:30例(WPV1:27、WPV3:3)であり、うち36ヶ月齢未満の小児は192 例(84%)、予防接種は68例が未接種、51例が1～3回、9例が4回以上の接種を受けていた。発症地域は157地域のうち60地域(38%)、主にKP、FATA地域、バローチスターン、シンド州にて多かった。
