2005年以降、Advisory Committee on Immunization Practices(ACIP)は青少年の定期予防接種スケジュールに11歳~12歳時の結合型髄膜炎菌ワクチン(MenACWY)の2回接種、破傷風/ジフテリア/無細胞性百日咳ワクチン(Tdap)の1回接種、ヒトパピローマウイルス(HPV)ワクチンの3回接種、インフルエンザワクチンの年1回接種を追加した。13~17歳の青少年における予防接種率を評価するため、CDCはNational Immunization Survey-Teen(NIS-Teen)のデータを分析した。2010年から2011年にかけて10歳以上でのTdap≧1回接種率(2010年:68.7%、2011年:78.2%)、MenACWY≧1回接種率(それぞれ62.7%、70.5%)、女性におけるHPVワクチンの≧1回接種率(48.7%、53.0%)、≧3回接種率(32.0%、34.8%)は上昇した。しかし女性におけるHPVワクチン≧1回接種率の上昇(4.3ポイント)は、TdapやMenACWY≧1回接種率の上昇(7.8~9.5ポイント)の約半分であった。HPVワクチンの接種率は人種/民族と貧困レベルによって異なり、男女とも≧1回接種率は黒人とラテンアメリカ系が白人よりも有意に高く、貧困レベル未満群が貧困レベル以上群よりも有意に高かった。しかし女性において、1回接種後の接種完了率は黒人が白人よりも有意に低く、貧困レベル未満群が貧困レベル以上群よりも有意に低かった。Healthy People 2020では13~15歳の青少年におけるTdap≧1回接種率、MenACWY≧1回接種率、女性におけるHPVワクチン≧3回接種率の目標を80.0%としているが、2011年の接種率はそれぞれ80.5%、71.5%、30.0%であった。州別ワクチン接種率は、Tdap≧1回接種率が36.9%(ミシシッピ州)~95.0%(ニューハンプシャー州)、MenACWY≧1回接種率が27.6%(アーカンソー州)~92.1%(インディアナ州)、女性におけるHPVワクチンの≧1回接種率が31.9%(ミシシッピ州)~76.1%(ロードアイランド州)、≧3回接種率が15.5%(アーカンソー州)~56.8%(ロードアイランド州)と様々であった。HPV関連癌から青少年を守るためには、医療従事者による強力な接種勧告、未接種者/リコールシステムの実施、青少年の親に対するHPV感染症のリスクとワクチン接種の有益性に関する教育などが必要である。
