Vital Signs: Risk for Overdose from Methadone Used for Pain Relief － United States, 1999-2010

バイタルサイン：疼痛軽減のために使用するメタドンの過量投与のリスク－アメリカ、1999～2010年

オピオイド系鎮痛薬(OPR)のメタドンはOPR過量投与による死亡の1/3に関連しているというデータがあることから、CDCは1999～2010年のメタドンの過量投与による死亡率と全国での販売量を調査した。またDrug Abuse Warning Network Medical Examiner Systemに報告された13州における2009年のOPR過量投与による死亡率を、メタドンと他の主要なオピオイドで比較した。アメリカにおいて、2009年のメタドンの過量投与による死亡率は1999年の5.5倍であった。メタドンの過量投与による死亡率は疼痛治療におけるメタドン処方の増加に伴って上昇し、2007年がピークとなった(1.8/10万人)。また疼痛治療におけるメタドンの年間処方率は、2008年に1.5/100人まで上昇した。2010年、メタドンは各州に分配されたオピオイドの4.5%(ニュージャージー州)～18.5%(ワシントン州)を占めた。2009年の13州におけるOPRに関連(他剤も併用)した死亡者(3,294例)の31.4%、OPR単剤が原因の死亡者(748例)の39.8%はメタドンが原因であり、メタドンの過量投与による死亡率は他のOPRの過量投与による死亡率よりも有意に高かった。メタドンは、OPRの有益性が証明されていない症状に対して不適切に処方されている。メタドンを含む徐放性オピオイドは軽度疼痛/急性疼痛/ブレイクスルー疼痛に対して、あるいは頓用として使用するべきではなく、また慢性非癌性疼痛に対してメタドンを最初に考慮するべきではない。メタドンは、その使用経験が豊富な医師が適切なオピオイド処方ガイドラインを順守して使用する必要がある。
