Multiple Outbreaks of Hepatitis B Virus Infection Related to Assisted Monitoring of Blood Glucose Among Residents of Assisted Living Facilities － Virginia, 2009-2011

日常生活動作支援介護付き集合住宅居住者における補助的血糖値モニタリングによるB型肝炎ウイルス感染症の複数のアウトブレイク－バージニア州、2009～2011年

2009年2月～2011年11月、Central Health Planning Region of Virginiaにある4ヵ所の日常生活動作支援介護付き集合住宅(ALFs)における急性B型肝炎ウイルス(HBV)感染の発生がVirginia Department of Health (VDH)に報告された。施設Aでは2009年2月、補助的血糖値モニタリング(AMBG)を受けた71歳男性が急性HBV感染症を発症、2009年5月、さらに1名での感染が確認された。HBVスクリーニング検査の結果、さらに2名での感染が確認され、5名の慢性患者が同定された。いずれもAMBGを受けており、ゲノム配列分析では99.9～100.0%一致し、患者間感染が示唆された。施設Bでは2010年1月、急性および慢性HBV感染が各1名認められ、スクリーニング検査の結果、さらに13名の急性HBV感染症と新たに4名の慢性患者が確認された。糖尿病を患う居住者19名のうち13名がAMBGを受けており、12名(92%)が急性HBV感染症を来した。ゲノム配列は99.8～100.0%一致し、AMBGに使用されたペンレット型の再利用可能フィンガースティック装置の共有使用が原因であることが示唆された。施設Cでは2010年6月、AMBGを受けた76歳男性が急性HBV感染症を来した。2010年10月のスクリーニング検査ではさらに2名が急性HBV感染症と診断され、うち1名は慢性HBVと判明、この症例は急性HBV感染症と診断された直後、施設Bより2009年10月に移動してきていた。スクリーニング検査の結果、さらに3例の感染が確認され、いずれも施設CにてAMBGを受けていた。また、施設Dにおいても2011年11月、55歳男性の急性HBV感染が認められ、スクリーニング検査の結果、6名の急性HBV感染、4名の慢性HBV感染が認められた。いずれの施設においても糖尿病治療のためのAMBGによる感染で、不適切な検査の実施によるものと判断され、検査スタッフの訓練の必要性が示唆された。
