Investigation of Viral Hepatitis Infections Possibly Associated with Health-Care Delivery － New York City, 2008-2011

医療介護供給に関連する肝炎ウイルス感染症の調査－ニューヨーク市、2008～2011年

ニューヨーク市のDepartment of Health and Mental Hygiene(DOHMH)による医療介護供給に関連するB型肝炎ウイルス(HBV)、C型肝炎ウイルス(HCV)感染の症例調査について報告する。症例1)2007年5月、NYC在住の60代男性。同年2月に結腸鏡検査を受け、32日後よりインフルエンザ様症状、食欲不振および関節痛を来した。臨床検査ではHCV RNAが確認され、肝酵素値の上昇が認められたことからC型肝炎と診断された。5月29日、結腸鏡検査を行った消化器医および麻酔医を調査し、麻酔に用いるプロポフォール(1回:6～8cc)を同一のバイアル( 20または50cc入)から複数の患者に投与していたことが明らかとなった。DOHMHはこの投与方法を中止するよう指導した。同じ日に麻酔下で結腸鏡検査を受けた他の6例のうち、2例でHCV感染が確認されたが、2例とも検査前からの感染例で、うち1例はこの男性の直前に食道胃十二指腸内視鏡検査(EGD)を受けており、HCV遺伝子型(2b型)およびウイルス配列が一致した。さらに、2005年1月1日～2007年11月30日にこの麻酔医から診療請求のあった2,907例の電子データベースと、DOHMHのHCVデータベースを照会した結果、同じ日に検査を受け、検査日以降にHCV感染が初めて診断された症例が少なくとも1例認められた。症例2)2006年4月にEGD検査を受けた40代女性。検査から5週間後に急性HCV感染症のため入院したが、この時点ではDOHMHへの報告は行われず、2009年4月に代理人から報告された。調査の結果、EGD検査は症例1と同じ麻酔医により行われ、同一バイアルから複数の患者へのプロポフォール投与を受けていたことが明らかとなった。DOHMHはこれら患者間感染がバイアル、針または注射器の不適切な仕様によるものと判断し、2005年1月1日～2008年7月30日の間にこの麻酔医の処置を受けた3,287例に対しHBV、HCVおよびHIV検査を受けるよう郵便にて推奨している。
