ヒトパピローマウイルスに関連する癌－アメリカ、2004～2008年

ヒトパピローマウイルス(HPV)はほとんど全ての子宮頸癌、および外陰部、膣、陰茎、肛門、口咽頭癌の発症に関与している。2004～2008年のNational Program of Cancer Registries (NPCR)およびSurveillance, Epidemiology, and End Result (SEER)データの分析では、この期間のHPV関連癌診断例は年平均33,369例(男性:12,080、女性:21,290、人口10万人あたり10.8)で、癌種別では子宮頸癌が最も多く年平均11,967例、次いで口咽頭癌:11,726例(男;9,356、女:2,370)、肛門癌:4,767例(男:1,678、女:3,089)で、口咽頭癌の発症率は男性が女性の約4倍高く(6.2 vs 1.4)、肛門癌は女性が高値であった(1.2 vs 1.8)。州別では女性がユタ州(10万人あたり8.5)～ウエストバージニア州(16.3)、男性がユタ州(4.9)～ワシントンDC(11.6)であった。CDCはHPVによる新規発症例は年間26,000例(男性:8,000、女性:18,000)、癌種別では子宮頸癌:11,500例、口咽頭癌:7,400例(男性:5,900、女性:1,500)と推定している。HPVに対しては2価および4価のワクチンが承認されており、Advisory Committee on Immunization Practicesは11～12歳の女児に対し2価または4価ワクチン、男児に対して4価ワクチンの3回接種、さらに女性は26歳、男性は21歳までのキャッチアップ接種を推奨している。2010年では13～17歳の女児の32%がHPVワクチンの3回接種を完了している。
