Measles － United States, 2011

アメリカでは2000年に麻疹を根絶しているが、その後も国外からの輸入感染は持続し、2001～2010年には年間60(37～140)例、4(2～10)アウトブレイクが報告され、2011年には222例(人口100万人あたり0.7)および17のアウトブレイクが報告されている。ここでは2011年の5月までの麻疹の発症状況について述べる。輸入感染は発疹の発現前7～21日に国外で麻疹ウイルスに曝露、帰国から21日以内に発疹の発現を認める症例とした。麻疹の発症は31州から222例(3ヵ月～84歳、中央値:14歳)報告され、200例(90%)は輸入感染に関連する症例で、うち52例(26%)は海外から帰国したアメリカ人、20例(10%)は海外からの観光客であった。アメリカ人196例のうち166例はワクチン未接種または接種状況不明例で、そのうち141例(85%)は接種適格例であった。入院した症例は70例で、下痢(17例)、脱水症状(15例)、肺炎(12例)などの症状を示し、脳炎を来した症例および死亡例は認められなかった。研究室レベルでの確診例は200例で、94例(47%)は麻疹特異IgMおよび麻疹ウイルス、69例(35%)はIgM、37例(19%)はウイルスのみ検出され、ウイスルの遺伝子型はD4、D9、D8、B3、G3およびH1の6種類であった。以上、麻疹の輸入感染の増加はワクチンの未接種によるものと考えられ、国外へ旅行する場合、6ヵ月齢以上の場合はMMRワクチンを接種すべきであり、12ヵ月齢以上の場合はMMRワクチンを28日以上空けて2回、接種すべきである。
