FDA Approval of an Extended Period for Administering VariZIG for Postexposure Prophylaxis of Varicella

痘の曝露後予防のためのVariZIGの投与可能期間延長に関するFDAの承認

アメリカにおいてVariZIGは、水痘に対する免疫のエビデンスがなく水痘ワクチンの接種適応外で重症化のリスクが高い患者における水痘の曝露後予防のために使用可能な唯一の水痘-帯状疱疹免疫グロブリン製剤である。2011年5月、FDAはVariZIGの投与可能期間を今までの水痘-帯状疱疹ウイルス曝露後96時間(4日)から10日に延長することを承認した。数件の研究データは、水痘-帯状疱疹免疫グロブリンを曝露後4日以内に投与した患者と5～10日目に投与した患者で水痘の発症率が同等であること、曝露後10日目までに免疫グロブリンを投与した場合には水痘を発症しても軽度であることを示唆する。Advisory Committee on Immunization Practices(ACIP)は、VariZIGの適応に関する勧告を変更していない。ACIPがVariZIGの投与を推奨している患者は、免疫不全患者、母親が周産期(出産の5日前から出産2日後まで)に水痘の徴候/症状を有していた新生児、新生児期に水痘-帯状疱疹ウイルスに曝露し母親が水痘に対する免疫のエビデンスがない早産児(＝妊娠28週に出生)、新生児期に水痘-帯状疱疹ウイルスに曝露した早産児(＜妊娠28週に出生、あるいは出生体重＜＝1,000g)である。
