Tuberculosis Outbreak Associated with a Homeless Shelter - Kane County, Illinois, 2007-2011
路上生活者のシェルターにおける結核アウトブレイク-イリノイ州ケーン郡、2007~2011年
アメリカでは結核(TB)の罹患率は減少しているが、路上生活者におけるTBアウトブレイクは依然として大きな問題である。イリノイ州ケーン郡では2007年4月、55歳の男性が喀痰塗抹陽性TBと診断され、2011年9月までに計28例の感染が認められた。この男性は路上生活者であり、約180名収容可能なケーン郡の短期滞在型シェルターに宿泊していた。その後、2009年10月および2010年1月に、このシェルターの宿泊客2名が同じ遺伝子型を示すM. tuberculosisによるTBを発症し、2010年3月までにさらに3例、2011年7月には計25例にて発症、いずれの症例もこのシェルターに宿泊し、同一の遺伝子型を示したことから、2011年9月よりこのアウトブレイクに関する調査が開始され、9月23日の時点で28例が確認された。小児1例を除いた27例の年齢は49(19~64)歳、感染源調査のため行われた症例対照試験では、シェルター長期滞在(250日以上)、アルコールの多飲、バーでの飲酒などとの関連性が示唆された。うち24例に直接監視下療法が行われ、12月までに完了または継続している。州および郡はシェルターでの感染を抑制するため、シェルター宿泊客に対しTB症状のスクリーニング、来客に対し10日以内および1年以内のTBおよび感染症の評価を行うなどの行政的な規制措置が実施された。調査開始後にさらに3例を認めたが、2011年12月以降2012年3月5日の時点で新たな症例は認めていない。路上生活者では過度の飲酒や過密なシェルターでの生活などTB感染リスクが高く、そのコントロールは継続的に行われるべきであり、アウトブレイクを認めない場合も検査および潜在性TB感染に対する治療を優先的に行うべきと考える。
