Updated Recommendations for Use of Tetanus Toxoid, Reduced Diphtheria Toxoid and Acellular Pertussis Vaccine (Tdap) in Pregnant Women and Persons Who Have or Anticipate Having Close Contact with an Infant Aged ＜12 Months - Advisory Committee on Immunization Practices (ACIP), 2011

生後12ヶ月未満の乳児はより年長の小児や成人と比べて百日咳の発症率がかなり高く、百日咳関連死の負担が最も大きい。2004年以降、毎年平均3,055例の百日咳乳児(死亡19例)がNational Notifiable Diseases Surveillance Systemを通して報告されている(CDC、2011年未発表データ)。百日咳患者および百日咳による入院・死亡患者の多くは生後2ヶ月以下の乳児であるが、この年齢群の乳児にはワクチンが接種できないため他の百日咳予防戦略が必要である。乳児の百日咳予防のため、Advisory Committee on Immunization Practices(ACIP)は2005年以降、ワクチンを接種していない出産後の母親やその他の新生児の家族に対して破傷風トキソイド+弱毒化ジフテリアトキソイド+無細胞性百日咳ワクチン(Tdap)の追加接種を勧告している(Cocooning[家庭優先]プログラム)。しかし過去5年間に出産後の母親のワクチン接種率はある程度上昇したが、父親や他の家族の接種率はそれほど上昇していない。そこで2011年6月22日、ACIPはワクチンを接種していない妊婦に対するTdap接種勧告を新たに作成し、Cocooningプログラムと特定の状況での接種勧告を更新した。この報告は、ACIPが考慮したデータ(Tdapの安全性、母親の抗体の経胎盤移行、ジフテリア+破傷風トキソイド+無細胞性百日咳ワクチン[DTaP]初回接種時の乳児の免疫反応に対する母親の抗百日咳ウイルス抗体の影響、費用対効果分析など)の概要と結論、勧告の内容について述べたものである。最新勧告によると、Tdapの接種歴のない妊婦は妊娠中、できれば妊娠後期あるいは妊娠中期の後半(妊娠20週以降)にTdapを接種するべきである。妊娠中に接種できなかった場合には、出産後すぐにTdapを接種すべきである。また生後12ヶ月未満の乳児と密接に接触する、あるいは密接な接触が予測されるTdap接種歴のない青少年と成人(親、兄弟姉妹、祖父母、保育士、医療従事者など)も、Tdapを接種するべきである(理想的には乳児との密接な接触が始まる少なくとも2週間前)。Tdapの接種歴がない妊婦で、妊娠中に破傷風+ジフテリアワクチン(Td)の追加接種が必要な場合(すなわち最後のTd接種から10年以上経過)、あるいは創傷管理のため破傷風トキソイド含有ワクチンの接種が必要な場合(最後のTd接種から5年以上経過)には Tdapを接種すべきである。破傷風ワクチンの未接種の妊婦は破傷風+弱毒化ジフテリアトキソイド含有ワクチンを3回接種する必要があるが(0週、4週後、6～12ヶ月後)、3回のうち1回、Tdの代わりにTdapを接種するべきである(できれば妊娠20週以降)。
