インフルエンザに関連する小児の死亡－アメリカ、2010年9月～2011年8月

アメリカでは2004年10月よりインフルエンザによる小児死亡例は報告義務が課されており、CDCは今回、2010年9月1日～2011年8月31日に報告されたインフルエンザ関連小児死亡例115例についての調査結果を報告した。性別では男児:72例(63%)、女児:43例(37%)、年齢中央値は6歳、53例(46%)が5歳未満であり、人種別では非ラテン系白人:52%、非ラテン系黒人:18%、ラテン系:15%であり、死亡時期は1月下旬から2月上旬にかけて最も多かった。ウイルスは2009インフルエンザA(H1N1):30例(26%)、A(H3N2):21例(18%)、サブタイプ不明のA型:20例(18%)、B型:44例(38%)であった。また、約半数(49%)の症例がAdvisory Committee on Immunization Practices(ACIP)が定めているハイリスク例ではなく、少なくとも1項目以上のハイリスク条件を満たす症例は57例(神経性障害:31、肺疾患:17、染色体異常または遺伝的障害:14、先天性心疾患または他の心疾患:11、喘息または反応性気道疾患:11)であった。74例(65%)は入院後に病院で死亡、20例(18%)は救急外来にて死亡、20例(18%)は病院外で死亡し、発症から死亡までは3日以内:33例(31%)、7日以内:69例(65%)であった。培養検査は25/64検体(39%)にて陽性であり、9例にてS.aureus（うち6例はメチシリン耐性）、6例にてS.pneumoniae、3例にてグループA streptococcusが検出された。ワクチン接種に関する情報が得られた6ヶ月齢以上の小児74例のうち、発症の少なくとも14日前に接種が完了していたのは17例(23%)であり、今後、ワクチンの接種率を向上させる必要性が示唆された。
