Influenza Vaccination Coverage Among Pregnant Women - United States, 2010-11 Influenza Season
妊婦におけるインフルエンザワクチン接種率-アメリカ、2010-11インフルエンザ期
妊娠中はインフルエンザの感染リスク、死亡リスクともに高くなるため、Advisory Committee on Immunization PracticesおよびAmerican College of Obstetricians and Gynecologistsは2004年より全ての妊婦に対しインフルエンザワクチンの接種を推奨している。今回、CDCは2010-11インフルエンザ期の妊婦におけるワクチン接種率について、インターネット調査を実施した。調査は2011年4月4~25日に、2010年10月~2011年1月の間に妊娠していた妊婦を対象に行われ、1,457名の回答を得た。ワクチンの接種率は49%(妊娠前の接種:12%、妊娠中:32%、出産後:5%)であり、年齢別では18~24歳:44%、25~45歳:52%、学歴別では大卒以上の妊婦にて高かった。62%の妊婦がかかりつけの医師からワクチンを接種するよう勧められ、その接種率は71%、勧められていない妊婦の接種率は14%であり、接種を勧められた妊婦は勧められていない妊婦に比べ接種に対し前向きであった[ワクチンの有効性(82%vs54%)、安全性(78%vs53%)、新生児に対する安全性(75%vs47%)]。接種を受けなかった理由は生まれてくる子供への安全性の不安(20%)が最も多く、接種によるインフルエンザ感染の不安(17%)、効果への不安(14%)、安全性の不安(11%)、インフルエンザに感染しても重症化しない/インフルエンザに感染した場合は薬を飲めばよい(14%)などが挙げられた。接種を受けた場所は、担当の産婦人科医または助産婦のオフィス(61%)、他の医師のオフィスや医療施設(22%)、薬局/食料品店(8%)、職場/学校(5%)であった。Healthy People 2020では妊婦におけるインフルエンザワクチン接種率の目標値は80%とされており、ワクチンの安全性と有効性を強く訴え、接種率の向上により妊婦および新生児の感染を予防することが重要と考える。
