Vital Signs: Hospital Practices to Support Breastfeeding － United States, 2007 and 2009

アメリカでは2～5歳児の21%が体重過多であり、その約半数は肥満である。小児肥満症の予防に対する母乳栄養の有効性について疫学的エビデンスが示されており、WHOとUnited Nation Children’s Fund (UNICEF)は、Baby-Friendly Hospital Initiativeを展開し、病院における母乳栄養のための10段階の指標を設定している。今回、2007年、2009年におけるこの10段階指標の実施状況についての分析が行われた。調査参加施設は2007年:2,690、2009年:2,672施設であり、回答率はともに82%であった。2007、2009年におけるそれぞれの指標の実施率は、出産前の妊婦への教育(2007年:92.5%、2009年:92.8%)、母乳の与え方の指導(それぞれ87.8、89.1%)、母乳を与えるタイミングの指導(それぞれ77.0、81.8%)は高かったが、母乳栄養対策に関する文書があると回答した施設は2007年:11.7%、2009年:14.4%と少なかった。また、満期産の健康新生児に対する調合ミルク、水、ブドウ糖液などの使用を制限する施設はそれぞれ20.6、21.5%に過ぎず、退院後のサポート体制が整っている施設は2007、2009年ともに26.8%であった。これら10指標のうち、3～5項目を満たす施設は2007年:60.5%、2009年:54.3%であり、9項目を満たす施設は2007年:2.4%、2009年:3.5%であり、10項目すべてを満たす施設は1%に満たなかった。また、これらの実施率は地域や病院の規模により異なり、地域別では北東部の施設にて高く、病院の規模が大きいほど高い傾向が認められた。小児肥満症の抑制には、この10段階指標の実践により母乳栄養の開始と継続をサポートすることが重要である。
