HIV Screening of Male Inmates During Prison Intake Medical Evaluation － Washington, 2006-2010

刑務所に収容されている男性の医学的評価へのHIVスクリーニングの取り入れ－ワシントン州、2006～2010年

ワシントン州の矯正省(Washington State Department of Corrections:WSDOC)は州内12ヶ所の刑務所を管轄している。これらの刑務所には約15,000人の男性受刑者が収容されており、毎年、約6,700人が入れ替わる。新たに収容される受刑者は到着から14日以内に医学的な検査を受け、HIV陽性の場合は感染を告知し、治療が行われる。2007年9月までは刑務所でのHIV検査は囚人の要求があった場合にのみ実施していたが、その後、囚人の同意が得られた場合は、参加選択(opt-in)にて医学的検査にHIVのスクリーニング検査を定期的に組み入れることとし、2010年3月には不参加選択(opt-out)とし、拒否した場合のみスクリーニング検査をしないこととした。その結果、年間の検査数は2007年9月までは360件、参加選択とした後は4,780件(新規収容者の約72%)、不参加選択への変更後は5,899件(同約90%)となった。検査数の増加に伴い、新規診断例も増加し、2007年9月までは1.8例/年であったが、参加選択としていた期間(30.5ヶ月間)では5.1例/年、不参加選択とした後(9.5ヶ月間)は7.6例/年となった。新規診断例19例の調査では、年齢20～58(平均:35)歳、アメリカンインディアン/アラスカ原住民:3(16%)、ラテン系:1(5%)、非ラテン系黒人:5(26%)、非ラテン系白人:10(53%)、異性愛者:8(42%)、静注ドラック使用者:6(32%)、男性同性愛者:4(21%)であり、診断時のCD4数は422(71～898)/mm3であり、＜500/mm3:9例、＜200/mm3:3例であった。以上、Opt-out HIVスクリーニングはopt-inスクリーニングに比べ検査数の増加および症例検出率が大きく向上していた。
