麻疹のアウトブレイクと根絶へ向けての進歩－アフリカ地域、2009～2010年

2008 年、WHOアフリカ地域(AFR)のMeasles Technical Advisory Group(TAG)は、2012年末を麻疹根絶の前段階とする目標を発表した。この目標は、AFRの46ヶ国において2000年と比較して麻疹による死亡率を98%以上低下させること、発症率を100万人あたり5例未満とすること、麻疹含有ワクチンの初回接種(MCV1)率を全国で90%、全ての地域で80%とし、追加接種活動(SIAs)による接種率を95%以上とすることを掲げている。2001～2008年のMCV1接種率は55%から79%に増加し、2009年は83%と報告されており、ブルキナファソ、ガンビア、モーリシャス、サントメプリンシペの4ヶ国では全ての地域で80%の接種率であった。また、2009～2010年に全国規模のSIAが31ヶ国にて実施されている。麻疹の症例数は2000年:502,102例から2008 年:37,162例へ減少し、2009年:83,464例、2010年:172,824例と増加に転じている。また、2010年には25ヶ国(63%)にて非麻疹性発熱性発疹疾患が10万人あたり2例以上報告され、29ヶ国(79%)にて血液検体が採取された麻疹の疑われる症例が1例以上の地域が80% であった。さらに100例の大規模アウトブレイクは2009～2010年に28ヶ国(61%)にて発生し、2008年の9ヶ国(20%)を大きく上回っている。ワクチン接種率の低かった年齢層での発症が増加し、MCVの2回目接種(MCV2)の早急な実施の必要性が示唆される。
