Vital Signs: Prevalence, Treatment, and Control of High Levels of Low-Density Lipoprotein Cholesterol － United States, 1999-2002 and 2005-2008

低密度リポタンパク質コレステロール(LDL-C)が高値の場合、冠動脈性心臓疾患(CHD)リスクが高い。National Cholesterol Education Program Adult Treatment Panel-IIIガイドラインでは、LDL-Cの治療目標値はCHD高、中、低リスク例にてそれぞれ＜100mg/dL、＜130mg /dL、＜160mg/dLとされている。2005～2008年におけるNational Health and Nutrition Examination Survey (NHANES)データでは、20歳以上の成人の33.5%(約7,100万人)がLDL-C高値であり、うち薬物療法は48.1%(約3,400万人) に行われ、LDL-Cがコントロールされている症例は33.2%(約2,300万人)に過ぎないと報告されている。年齢別の罹患率は20～39 歳:11.7%、40～64歳:41.2%、65歳以上:58.2%であり、薬物療法は65歳以上にて64.4%、Medicare加入者にて63.4% が受けていた。コントロール率は前年の受診回数が2回以下(17.7%)および健康保険未加入者(13.5%)にて低かったが、LDL-Cがコントロールされていない症例の82.8%は健康保険に加入していた。1999～2002年のデータと比較した場合、罹患率は34.5%から33.5%へ減少、薬物療法を受けている症例は28.4%から48.1%、コントロール率は14.6%から33.2%への増加であった。LDL-C高値のコントロールは、医療機関の受診回数のみではなく、治療プログラムの改善や患者の治療遵守も重要な要素であると考える。
