State Medicaid Coverage for Tobacco-Dependence Treatments － United States, 2009

タバコ依存症治療に対する州のメディケイド補償範囲－アメリカ、2009年

メディケイド加入者は、一般人と比べ喫煙率が約2倍高く(37% vs 21%)、支出の約11%を喫煙関連の医療費が占めている。2008年、Public Health Serviceはタバコ依存症の薬物治療およびカウンセリングの費用を包括的に補償するようガイドラインを発表し、Healthy People 2010でもFDAが承認する薬物療法と禁煙カウンセリングを保険適用にすることが目標とされている。今回、CDCはUC Berkeleyとともに2009年の国内50州およびワシントンDCにおけるタバコ依存症治療に対するメディケイドの補償範囲に関する調査を行った。その結果、全51プログラムのうち4プログラム(コネチカット、ジョージア、ミズーリ、テネシー州)はタバコ依存症治療に関する補償が全くなく、47プログラムにて受給者の一部に対しタバコ依存症治療を補償し、38プログラムでは全ての受給者に対し少なくとも1つのタバコ依存症治療を補償していた。補償する内容はニコチンパッチ(34プログラム)、ブプロピオンまたはZyban(33)、ニコチンガム(32)、バレニクリン(32)、ニコチン経鼻スプレー(28)、ニコチン吸入薬(27)、ニコチントローチ錠(25)であり、カウンセリングに関しては薬物治療に比べ補償するプログラムは少なく、個人カウンセリングは18プログラムにて加入者全員に、6プログラムにてFFS(出来高払い型)、1プログラムにてMCO(管理医療型)、6プログラムにて妊婦を対象に補償し、グループカウンセリングは全員:8、FFS:3、MCO:1、妊婦:5プログラムにて補償していた。また、5州(インディアナ、マサチューセッツ、ミネソタ、モンタナ、ペンシルバニア州)が全てのメディケイド加入者に対し、薬物療法、カウンセリング(個人およびグループ)を補償していた。2007年の調査以降、ネブラスカ州とアラバマ州にて補償が開始、計12プログラムにて補償の追加や拡大されていた。
