Rotavirus Vaccination Coverage Among Infants Aged 5 Months － Immunization Information System Sentinel Sites, United States, June 2006-June 2009

2006 年2月、Advisory Committee on Immunization Practice(ACIP)はアメリカの全ての乳児に対し、生後2、4、6ヶ月において5価ロタウイルスワクチン(RV)を3回接種するよう勧告を出し、2008年6月には2回目の接種を生後2、4ヶ月に1価ワクチンを2回接種するよう更新した。1回目の接種は生後14週と6日までとしている。CDC はImmunization Information System(IIS) Sentinel Siteデータから、1)2006年6月～2009年6月における5ヵ月齢の乳児における1回以上のRV接種率、2)2009年第2四半期におけるRV接種率と他のワクチン[ジフテリア、破傷風、無細胞性百日ぜき(DTaP)ワクチン、7価肺炎球菌結合型ワクチ（PCV7)]との比較に関して分析した。 2008～20012年のIISのSentinel　Siteは、アリゾナ、コロラド、ミシガン、ミネソタ、オレゴンおよびウイスコンシン州の一部とノースダコタ州、ニューヨーク市全域の計8地域であり、2009年6月30日現在のデータでは、参加する乳児は23,532名(164～11,767名/地域)、RVワクチン接種率は平均72%(コロラド州:48%～ノースダコタ州:86%)であった。1回以上のDTaP接種率は85%(ニューヨーク市:71%～ノースダコタ州:93%)、PCV7接種率は85%(ニューヨーク市:72%～ミシガン州:91%)であった。コロラド州ではRV接種率と DTaP/PCV7接種率の差がともに37%と最も大きく、ミネソタ州ではDTaP接種率との差が2%、ノースダコタ州ではPCV7接種率との差が4%と最も小さかった。また、2004～2007年にもIIS Sentinel　Siteであったアリゾナ、ミシガン、ミネソタ、オレゴン州では、勧告の発表から1年後には50～60%となり、その後、1四半期ごとに2.7%増加し、2009年第2四半期には74%となっている。ワクチン接種の導入により2007-08年、2008-09年におけるロタウイルスの感染期は前年に比べ短くなり、発症例の重症度も軽減しているため、今後も接種状況をモニターし、接種の拡大を推進していきたい。
