Invasive Pneumococcal Disease in Young Children Before Licensure of 13-Valent Pneumococcal Conjugate Vaccine - United States, 2007
13価肺炎球菌結合型ワクチン認可前の幼児における浸潤性肺炎球菌疾患-アメリカ、2007年
アメリカでは2000年に開始された7価肺炎球菌結合型ワクチン(PCV7)の接種により、5歳未満の幼児における浸潤性肺炎球菌疾患(IPD)の発症率は 76%減少しているが、非PCV7血清型肺炎球菌によるIPDの発症率は増加している。2010年2月、Advisory Committee on Immunization Practices(ACIP)は新たに認可された13価肺炎球菌結合型ワクチン[PCV13:PCV7が含有する血清型(4、6B、9V、14、 18C、19F、23F)と新たに1、3、5、6A、7F、19Aの6種の血清型を含む]の接種に関する勧告を発表している。ここではCDCによる PCV13認可前(2007年)における5歳未満のIPD幼児例に関するデータ(Active Bacterial Core surveillance:ABCs)の分析結果を報告する。2006~2007年はコネチカット、ミネソタ、ニューメキシコ州およびカリフォルニア、コロラド、ジョージア、メリーランド、ニューヨーク、オレゴン、テネシー州の一部の郡にてIPDサーベイランスが行われ、5歳未満の幼児493例にてIPD の発症が確認された。年齢別発症数は12ヵ月未満:155(36%)、12~23ヶ月:124(29%)、24~35ヶ月:71(17%)、36~47ヶ月:48(11%)、48~59ヶ月:29(7%)であり、人種別の発症率は10万人あたり黒人:35.8、他民族:30.7、白人:18.4であった。血清型は427例にて明らかであり、PCV13に含まれる血清型による発症が274例(64%)を占め、260例はPCV7に含まれない血清型(3、 7F、19A)によるものであり、19Aによる発症数は180例(42%)と最も多かった。入院および臨床転帰に関しては272例にて明らかであり、 168例(62%)が入院し、4例(2%)が死亡した。これらのデータから、アメリカにおける発症数は4,600例、PCV7血清型により70例、 PCV13血清型により2,900例と推定された。今後、PCV13の導入によりさらにIPD発症数は減少することが期待され、認可後のモニタリングによりその有効性および非PCV13血清型によるIPD発症数の変化が明らかになると思われる。
