Idiopathic Granulomatous Mastitis in Hispanic Women － Indiana, 2006-2008

ラテン系女性における特発性肉芽腫性乳腺炎－インディアナ州、2006～2008年

特発性肉芽腫性乳腺炎(IGM)は妊娠可能な年齢の女性において、乳房域に発生する稀な感染症であり、世界で数百例しか報告されていないが、2008年12 月12日、インディアナ州インディアナポリスの医師から、経産婦ラテン系女性にてIGMを7例(26～35歳、うち6例がメキシコ出身)に認めたとの報告があった。この7例は乳房に痛みのある触診可能なしこりがあり受診(6例は片側、1例は両側)、いずれも経産婦であり、症状は最終妊娠の 34(10～62)ヶ月後、授乳終了から26(4～46)ヶ月後に発現しており、症状の発現から診断までは5(3～6)ヶ月経過していた。CDCにおいて生検が行われ、全例がIGMと診断され、5例が抗生物質投与を受け、3例が切除術を受けた。また、6例が診断前3年以内にツベルクリン検査(TST)を受けており、5例が陽性、4例がBCGの接種を受けていた。CDCおよびIndiana State Department of Healthによる症例対照試験(対照例:21、22～37歳)では、IGM症例は対照例に比べ教育レベルが低く、薬物アレルギーおよびTST陽性例が多く認められた。インディアナポリスにおける2006～2008年におけるIGM症例は9例(ラテン系:8、白人:1)であり、年間発症率は20～40歳の女性10万人あたり2.4、ラテン系女性では28.3と約12倍高値であった。今回、ラテン系女性においてIGMが多数発症した理由については明らかではなく、今後、さらに人種的、社会的要因を含めたIGMの疫学および病因について調査していきたい。
