Receipt of Influenza Vaccine During Pregnancy Among Women With Live Births － Georgia and Rhode Island, 2004-2007

出産した女性における妊娠中のインフルエンザワクチンのレセプト－ジョージア、ロードアイランド州、2004～2007年

妊娠女性はインフルエンザ合併症のリスクが高く、Advisory Committee on Immunization Practices(ACIP)およびAmerican College of Obstetric and Gynecologist(ACOG)Committee on Obstetric Practiceは2004年より全ての妊婦に対して3価不活性化ワクチンの接種を推奨している。今回、CDCによりジョージア州およびロードアイランド州のPregnancy Risk Assessment and Monitoring System(PRAMS)データから、妊婦におけるワクチン接種率の調査が行われた。調査対象はジョージア州:2004～2006年、計5,231名 (回答率:2004年:70.0%、2005年:70.2%、2006年:70.8%)、ロードアイランド州:2004～2007年、計5,499名(回答率:2004年:75.5%、2005年:75.1%、2006年:72.5%、2007年:72.1%)であり、接種率は両州ともに2004年から 2005年にかけて最も増加し(ジョージア州:10.4%から16.9%へ62.5%増、ロードアイランド州:21.9%から30.1%へ37.4% 増)、2006年には横這い、2007年にはロードアイランド州では10.0%増加していた。ロードアイランド州ではインフルエンザワクチンについての助言を受けたり、接種を勧められた妊婦は2004年:33.0%から2007年:47.7%に増加しており、2007年、助言を受けた妊婦の65.7%が実際に接種を受け、助言を受けなかった妊婦の接種率は4.6%であった。ジョージア州では接種を受けなかった理由として、いつも受けていなかった (69.4%)、医師が妊娠中のワクチン接種について何も言わなかった(44.5%)、胎児への安全性が懸念される(28.1%)、母体への安全性が懸念される(27.1%)などが挙げられた。今後も妊婦のワクチン接種率を向上させるため、医療提供者および妊婦に対しACIPおよびACOGの勧告について教育していくことが必要であろう。
