Use of Northern Hemisphere Influenza Vaccines by Travelers to the Southern Hemisphere

南半球への渡航者における北半球用インフルエンザワクチンの使用

温帯におけるインフルエンザシーズンは北半球では10月～3月、南半球では4月～9月である。また熱帯ではインフルエンザウイルスは1年中循環することが可能であり、熱帯および亜熱帯の国々への渡航者においてインフルエンザは最も発現頻度の高いワクチン予防可能な疾患となっている。旅行中のインフルエンザのリスクを低下させるため、米国予防接種諮問委員会(ACIP)は北半球からの旅行者のうち年1回のワクチン接種を推奨されている者やインフルエンザ疾患にかかりたくないが2008-09年インフルエンザワクチンを接種していない者に対し、次の場合にワクチン接種を考慮するよう勧告している。1)インフルエンザシーズン中の南半球への旅行前、2)時期に関係なく熱帯地域への旅行前、3)4月～9月にインフルエンザウイルスが循環している地域(南半球など)からの参加者を含む団体旅行の場合。アメリカでは南半球用のインフルエンザワクチンは入手できない場合が多いため、南半球への旅行者に対し北半球用に調整されたワクチンを接種する。現在、アメリカで入手可能な2008-09北半球用インフルエンザワクチンに含まれるインフルエンザウイルス株は、南半球で 2009年インフルエンザワクチンに含めようとしている株と同一である。したがって、医療提供者は旅行計画を尋ね、旅行中のインフルエンザのリスクに関する情報を提供し、現在入手可能な北半球用インフルエンザワクチンの接種が最新の南半球用ワクチンを提供することになるという可能性を知らせる必要がある。アメリカにおける北半球用インフルエンザワクチンの一部のロットは2009年6月30日まで使用可能である。インフルエンザワクチンは出発日まで接種可能であるが、可能ならば少なくとも出発の2週間前に接種すべきである。
