幼児5名における浸潤性B型Haemophilus influenza疾患－ミネソタ州、2008年

2008 年、Minnesota Department of Health(MDH)に5歳未満の幼児5名が浸潤性b型Haemophilus influenza(Hib)に感染し、うち1名が死亡したと報告された。症例は5ヶ月～3歳、うち髄膜炎を発症した7ヶ月齢の乳児が死亡した。Hibワクチンは5名中1名のみ完全接種、3名 (死亡例を含む)は1回も接種を受けていなかった。ミネソタ州では5歳未満の幼児での5例の発症は1992年以降最も多い数字であるが、時期的には 2007年12月よりMerck社がHibワクチン(2種類)を回収、製造を中止し、Hibワクチン製造はSanofi社のみとなり、全国的にワクチン供給が不足した状況での発症であった。これを受けてCDCは医療関係者に12～15ヶ月齢にて行う追加接種をHib疾患リスクがない場合は延期するよう指示、初回接種は行うよう強調したが、ミネソタ州では2008年の初回接種率が前年に比べ低く、2007年11月1日～2008年3月31日に出生した小児 25,699名のデータでは、7ヶ月齢における3回接種率は46.5%であった。三種混合(DTaP)、肺炎球菌ワクチン(PCV)接種率は全国平均と同等であり、Hibワクチン接種率はワクチン不足により低下したと考えられる。現在のワクチン供給は初回接種を保証するに十分な状況であるが、追加接種にはまだ不足しているため、MDHおよびCDCは効率的なワクチン供給と接種方法についての検討を進めている。
