65歳未満の石綿症関連損失生存可能年数-アメリカ、1968~2005年
アスベスト繊維の曝露は石綿症などの疾患を誘発する可能性があり、初回曝露から発症までの潜伏期間は10~40年である。米国においてアスベストは国内で製造される一部の製品に未だに使用されており(2006年:2200トン)、海外で製造されたアスベスト含有製品も輸入され続けている。米国では建物の修理や破壊中のアスベスト処理などにより毎年建設業および一般企業の労働者約130万人がアスベストに曝露されている可能性がある。また米国ではアスベスト鉱物は採掘されないが、鉱山労働者の一部は他の鉱物に含まれているアスベストに曝露されるリスクがある。米国における石綿症による早期死亡の特徴を調査するため、CDCは1968~2005年の年間基礎死因データを分析し65歳未満の石綿症関連損失生存可能年数(YPLL)を評価した。この期間中、石綿症を死因としていたのは9,024例であった。このうち1,169例(13.0%)が25~64歳であり(25~34歳:1例[0.1%]、35~44 歳:17例[1.5%]、45~54歳:165例[14.1%]、55~64歳:986例[84.3%])、総YPLLは7,267であった(死亡者1例あたりの平均YPLL:6.2)。これら死亡者の多くは男性(1,125例[96.2%])、白人(1,064例[91.0%])であり、それぞれの YPLLは7,038(96.8%)、6,470(89.0%)であった。65歳未満の石綿症関連YPLL(1年あたり)は、1968~1972年の平均 146.0から2001~2005年には239.6へと64%増加した。YPLLは毎年変動し、1973年が最低(69)、1990年が最高(306)であった。州別ではテキサス州(1985年:577YPLL)、ペンシルベニア州(1999年:544YPLL)、ニュージャージー州(1990 年:527YPLL)、カリフォルニア州(1976年:468YPLL)が全死亡者の29.9%を占めた。1985~1999年に石綿症を死因とした 25~64歳の死亡者531例のうち153例(28.8%)で産業(54種)や作業情報(59種)が得られた。YPLLが高かった産業は建設業 (244YPLL)、船やボートの製造・修理、(41YPLL)、軍隊(41YPLL)、作業は断熱材関連(112YPLL)や配管工など (42YPLL)であった。今後も石綿症の予防、追跡、根絶のための努力を続ける必要がある。
