Rotavirus Surveillance - Worldwide, 2001-2008
ロタウイルスサーベイランス-世界、2001~2008年
ロタウイルス感染症は小児の重症急性下痢症を引き起こす。毎年、ロタウイルス下痢症により5歳未満の小児527,000例が死亡していると推定され、このうち85%以上はアフリカ、アジアの低所得国の小児である。ロタウイルスワクチンは2種類が承認されており、その有効性は85~98%、アメリカ、ヨーロッパおよびオーストラリアなどの11ヶ国にて定期予防接種に導入されている。現在、アフリカ、アジアの低所得国での臨床試験が行われており、その結果は 1~2年後に発表される予定である。ここでは2001~2008年のWHOによる世界的予防接種対策に基づくサーベイランスデータから、ロタウイルスによる小児下痢症に関する調査結果をまとめる。この期間、ロタウイルス検査が行われた5歳未満の急性下痢症による入院例は62,584例(アフリカ:4,356、アメリカ:26,065、ヨーロッパ:3,374、東地中海諸国:17,291、東南アジア/西太平洋諸国:11,498)であり、ロタウイルス検出率は40%(アフリカ:41%、アメリカ:34%、ヨーロッパ:40%、東地中海諸国:40%、東南アジア/西太平洋諸国:45%)であった。血清型の分析は4,936検体(アフリカ:325、アメリカ:388、ヨーロッパ:323、東地中海諸国:1,290、東南アジア/西太平洋諸国:2,610)にて行われ、アメリカ、ヨーロッパ、東南アジア/西太平洋諸国ではG1P(8)、G9P(8)、G2P(4)の3種で約2/3を占めたが、アフリカ、東地中海諸国ではGおよびP型に属さず、その他に分類される血清型がそれぞれ46%、50%を占めていた。これらの結果は今後、予防接種の効果を評価する上で重要な基本情報であり、ワクチンの疫学的研究にも有用である。アジア、アフリカの低所得国に対してはGAVI Alliance(Glovbal Alliance for Vaccines and Immunizations)による経済的支援が行われ、ワクチンの有効性が確認された後は支援の対象国がさらに拡大される予定である。
