Licensure of a Diphtheria and Tetanus Toxoids and Acellular Pertussis Adsorbed, Inactivated Poliovirus, and Haemophilusb Conjugate Vaccine and Guidance for Use in Infants and Children

2008 年6月20日、FDAはジフテリア・破傷風トキソイド・無細胞性百日咳吸着(DTaP)・不活性化ポリオウイルス(IPV)ワクチンと結合型b型インフルエンザ菌ワクチン(Hib)(破傷風トキソイド[TT]結合)の混合ワクチン(DTaP-IPV/Hib[Pentacel])を認可した。この報告は、 Pentacelの適応とAdvisory Committee on Immunization practice (ACIP)が示したその使用手引を紹介する。比較試験では、局所および全身性有害事象と重篤な有害事象の発現頻度はDTaP-IPV/Hib とDTaP、IPV、Hibを別々に接取した場合とで同等であり、DTaP-IPV/Hibの3、4回接種後の免疫原性も別々に接取した場合と同等であることが報告されている。DTaP-IPV/Hibは乳児および小児に対し生後2、4、6、15～18ヶ月の4回接種として使用する。一部のアメリカンインディアン/アラスカ原住民(AI/AN)小児は特に生後6ヶ月間 Hib疾患のリスクが高い。さらに同小児ではHib結合型ワクチン製剤により免疫反応が異なる可能性があり、ポリリボシルリビトールリン酸-髄膜炎菌外膜蛋白質(PRP-OMP)含有Hibワクチン製剤は他のHib結合型ワクチン(Hib-TTなど)よりも迅速に生後6ヶ月以内の予防抗体濃度へのセロコンバージョンをもたらす。したがって、AI/AN小児にはPRP-OMP含有Hibワクチンのみを使用する必要がある。
