Trends in HIV-and STD-Related Risk Behaviors Among High School Students － United States, 1991-2007

高校生におけるHIVおよびSTDに関連するリスク行為の傾向－アメリカ、1991－2007年

避妊手段を用いない性的交際または静注薬物の使用はHIV感染および性行為感染症(STD)リスクが高い。アメリカの高校生におけるHIV、STD感染に関連するリスク行為に関しては、1991－2005年の調査結果が報告されているが、今回、CDCにより2007年までのYouth Risk Behavior Surveys(YRBS)データが分析され、1991－2007年の調査結果が報告された。対象は国内50州およびDCの9－12年生であり、回答数は10,904－16,296件、学校単位の回答率は70－81%、生徒回答率は83－90%、全体での回答率は60－70%であった。1991－2007年において、性体験率は全体で12%減少(1991年:54.1%、2007年:47.8%)、複数の交際相手(4人以上)がいる生徒は20%減少(1991年:18.7%、2007年:14.9%)、現在の性体験率は7%減少(1991年:37.5%、2007年:35.0%)、避妊具の使用率は33%増加(1991年:46.2%、2007年:61.5%)していたが、2005－2007年においては性別、学年別、人種別いずれのサブグループにおいても有意な変動は認めなかった。静注薬物の使用率は4%以下を維持したが(1995年:2.1%、2007年:2.0%)、黒人およびラテン系の生徒では増加(黒人:1995年1.1%、2007年1.8%、ラテン系:1995年2.2%、2007年:3.1%)、また、2005－2007年にかけては10年生にて減少したが(2005年:2.3%、2007年:1.4%)、他のサブグループでは有意な変動は認めなかった。15－19歳におけるHIV、STD感染症の発症率および出産率は増加傾向にあると報告されていることから、これらの危険行為による健康上ノアウトカムに関するプログラムや活動は、生徒本人とともに家族、学校、医療機関、政府関係機関なども共に取り組んでいく必要が示唆される。
