Automated Detection and Reporting of Notifiable Diseases Using Electronic Medical Records Versus Passive Surveillance - Massachusetts, June 2006-July 2007

電子カルテ(EMR)システムは従来の臨床医による報告システムや研究室ベースの電子報告システムの欠点を改良した、情報を収集、分析し、報告することができるシステムであり、届出伝染病に関してもよりタイムリーで正確な臨床的調査が可能となる。マサチューセッツ州ではCDCのCenter of Excellence in Public Health Informatics at Harvard of Pilgrim Health Care、Children’s Hospital in Bostonの2機関の援助を受け、Massachusetts Department of Health、Department of Ambulatory Care and Prevention at Harvard Medical School、Harvard Pilgrim Health Care、Atrius Healthの4機関が共同でElectronic Support for Public Health(ESP)と呼ばれるシステムを設立し、2007年1月より始動している。データは2006年6月から収集されており、患者背景、バイタル、検査指示および結果、処方薬、診断コードなどの詳細なEMRデータがESPに転送され、クラミジア感染症、淋病、骨盤内炎症性疾患および急性A型肝炎に関しては定期的な分析が行われている。2006年6月~2007年7月、ESPにはクラミジア:758例、淋病:95例、骨盤内炎症性疾患:20例、急性A型肝炎:4例が報告され、現存の受動的調査システム(紙ベースの記録)に比べクラミジア感染症は545例から758例(39%)、淋病は62例から95例(53%)、骨盤内炎症性感染症は0例から20例、急性A型肝炎は1例から4例に増加した。また、患者の妊娠状態に関するデータが収集され、受動的調査では認められなかった妊娠症例がクラミジアおよび淋病患者にて81例報告された。ESPは現在、急性B型、C型肝炎および結核に関する報告システムが追加され、さらに、異なるEMRにて収集されたデータが地域の保険情報システムへの入力によりESPへインストールされている。今後、標準検査命名法の採用や治療リストの統一、報告項目の標準化などにより、さらに利用価値の高いシステムへと発展するであろう。
