妊娠中のサイトメガロウイルス感染症に関する産科および婦人科医の知識と診療の実際－アメリカ、2007年

アメリカにおいて先天性サイトメガロウイルス(CMV)感染症は出生児150名中約1名の割合でみられ、生産児750名中約1名は身体障害(聴覚障害、視覚障害など)を生じる。妊婦においてCMVは特に若年小児由来の感染体液(尿や唾液など)との接触を介して伝播する。利用可能なワクチンはなく治療選択肢は限られているため、CDCとAmerican College of Obstetricians and Gynecologists(ACOG)は妊婦間でのCMV感染予防対策として、産科医および婦人科医(OB/GYN)は妊婦にCMVに感染した可能性のある物品(おむつなど)は注意深く取り扱うこと、若年小児と接触後には徹底的な手洗いを行うことなどを助言するよう勧告している。このような努力にもかかわらず、CMV感染症とその予防法に対する女性の意識は低い。2007年3月‐5月、ACOGはOB/GYNの全国的サンプル305名を対象としCMV感染予防に関する知識と予防の実態について調査した。妊娠中のCMV感染リスクを低下させる行動としてOB/GYNの90%は手洗い、57%は器具を共有しないこと、55%は眼や口に小児の唾液がつかないようにすることを知っていた。しかし、患者にCMV感染予防に関するカウンセリングを行っていると回答したのは44%にすぎなかった。カウンセリングの内容として60%は手洗い、31%は器具を共有しないこと、30%は小児の唾液がつかないようにすることを助言していた。27%は2003年以降CMV感染妊婦を診療した経験があった。また、86%は妊娠中にCMV検査を行ったことがあり、その多くは患者の希望、あるいは妊娠中のCMV検査は単核細胞症様疾患の発見など一部の状況下で実施することというACOGおよびCDCの勧告に準じて胎児異常の診断のためのみに検査を実施していた。今後、CMV感染予防に関するOB/GYNのさらなるトレーニングと、CMV伝播に関する医師の知識が患者のカウンセリングにつながらない理由をより良く理解することが必要である。
