結腸直腸癌検査の利用－メリーランド、2002～2006年

2002年－2004年、メリーランド州は結腸直腸癌（CRC）による死亡率が米国50州+ワシントンDC中13番目に高かった。米国癌学会（ACS）やU.S. Preventive Services Task Forceなどは50歳（CRCリスクが高い場合は50歳以前）からCRCスクリーニング検査として便潜血試験（FOBT）・便免疫化学試験（FIT）（毎年）、S状結腸鏡検査（5年毎）、FOBT・FIT（毎年）+S状結腸鏡検査（5年毎）、バリウム注腸二重造影検査（DCBE）（5年毎）、結腸鏡検査（10年毎）を受けはじめるよう推奨している。2002年、Maryland Department of Health and Mental HygieneはCRCを含む7種類の癌の有病率とリスク行動を評価するためMaryland Cancer Survey（MCS）を開始した。また包括的癌コントロールプラン（MCCCP）では、50歳以上のメリーランド州住民における2008年までの目標値としてCRC検査非受検率15%以下、CRC検査受検率73%以上、結腸鏡検査（過去10年間）あるいはFOBT（過去1年間）+S状結腸鏡検査（過去5年間）の受検率57%以上を設定した。この報告では、2002年、2004年、2006年に行われたMCSの結果に基づきCRC検査の受検傾向を示す。50歳以上のメリーランド州住民において、CRC検査非受検率は2002年の25.9%から2006年には19.8%まで有意に低下した。またCRC検査受検率は2002年の63.8%から2006年には69.2%まで増加し、結腸鏡検査（過去10年間）あるいはFOBT（過去1年間）+S状結腸鏡検査（過去5年間）の受検率は2002年の46.5%から2006年には60.4%となり2008年目標値をすでに超えた。しかしメリーランド州住民のうち健康保険未加入者や最近健康診断を受けていない者は、人種にかかわらずCRC検査の受検率が有意に低かった。今後、少数民族や医療サービスが行き届かないグループにおけるCRC受検率を増加させるための対策が必要である。
