HIV Transmission Among Male Inmates in a State Prison System － Georgia, 1992-2005

州刑務所における男性囚人のHIV感染－ジョージア州、1992～2005年

刑務所の囚人におけるHIV感染率はアメリカ全体の約5倍高値である（2.0% vs. 0.43%）。Georgia Department of Corrections（GDC）では1988年より、主に収容される前の囚人および臨床的に感染が疑われる症例を対象に、HIV検査を実施しており、今回、2005年2月までの調査結果がまとめられた。2005年10月現在、ジョージア州では73施設にて44,990名の男性囚人が収容されており［15-88歳、黒人:28,350（63%）、白人:16,364（36%）、アメリカンインディアン:50（0.1%）、アジア系:47（0.1%）、不明:179（0.1%）］、HIV感染は856例（1.9%）、うち780例（91%）は収容前に感染が確認されており、732例（86%）は黒人であった。1988年7月－2005年2月の間に刑務所内で感染した症例は88例［21-58歳、黒人:59（67%）、白人:29（33%）］、診断された時期は1992年9月－2003年6月:47例（53%）、2003年7月－2005年2月:41例（47%）であった。出所した11例および死亡した2例を除く75例のうち、68例を対象に症例対照研究が行われ、多変量ロジスティック解析によりHIV感染と刑務所内での男性同性愛、刑務所内での刺青、BMI＜＝25.4kg/m2（収容時）および黒人との関連が認められた。刑務所内での男性同性愛を行った54例（症例:45例、対照:9例）のうち39例（それぞれ35例、4例）は収容前6ヶ月間に同性愛経験はなく、また、39例（72%）は合意していたと報告し、48例（89%）は複数の相手と関係していた。コンドームやその場しのぎの避妊具（ゴム手袋、ラップなど）は、合意の上での性行為の場合13例（30%）が使用し、金銭/食料/煙草との交換性行為の場合は3例（21%）がコンドームを使用せずゴム手袋、ラップなどを使用していた。刺青による症例の場合、59/68例（87%）が使用する器具を洗浄しており、うち52例（76%）では漂白処理しており、未洗浄の器具の使用は2例（3%）であった。HIV感染は避妊手段なしの性行為、刺青針の共用および血液感染により感染したと考えられた。
