Guillain-Barre Syndrome Among Recipients of MenactraR Meningococcal Conjugate Vaccine - United States, June-July 2005

Menactra(結合型髄膜炎菌ワクチン)接種者におけるギランバレー症候群－アメリカ、2005年6～7月

2005年1月14日、アメリカにてMenactra［4価(A、C、Y、W135)結合型髄膜炎菌ワクチン:MCV4］が認可された。2月、Advisory Committee on Immunization Practice(ACIP)は11－12歳の小児に対し健康診断時の接種を推奨し、まだ接種を受けていない15歳前後の小児は高校入学前に接種するよう勧告を出した。その後、2005年10月4日までにVaccine Adverse Event Reporting System(VAERS)にMCV4接種によるギランバレー症候群(GBS)5例が報告され、CDC、FDAとともにワクチンの安全性に関する調査システムが設立された。患者は17－18歳、男性2例、女性3例、ペンシルベニア州:2例、ニューヨーク、オハイオ、ニュージャージー州:各1例であった。6月10日－7月25日の間に接種を受け、14－31日後に手足の刺痛、歩行困難、筋力低下、脚の知覚麻痺などの症状を発症していた。11－19歳におけるGBS発症率は100,000人年あたり1－2例であり、MCV4の臨床試験では約7,000例に接種されたが、GBS発症例は認められていない。CDCはVaccine Safety Databox(VSD)に登録されたMCV4接種例11万例のデータから6週間以上の追跡データのある症例を分析した。結果、MCV4とGBS発症との関連性は否定できないが、データが不十分であるため、暫定的にGBSリスク症例に対しMCV4の接種を控えるよう勧告を出した。
