Council of State and Teritorial Epidemiologists(CSTE)は、2001年11月、国内各州および地域の保健局の疫学的能力の調査を行い、2002年には約10億ドルの予算がテロおよび緊急事態対策のため分配されている。調査結果は2003年に発表され、感染症および慢性疾患を除いた疫学的プログラムの不備が指摘されている。CSTEは2004年4－5月、国内5州(フロリダ、ミシガン、ニューヨーク、ノースカロライナ、テネシー)において、これらについてパイロット追跡調査を実施し、5－9月に全州および地域の保健局に対し、オンラインにて情報を配信した。8つのキープログラムを全て実施するには推定で3,790名が必要とされているが、2004年、州および地域保健局で働く疫学者は2,580名であった。プログラム別の人員不足は感染症疾患では現在の926名に対し、325名(35.1%)の増員が必要とされ、以下、慢性疾患［390、183(46.9%)］、環境衛生［324、164(50.6%)］、テロおよび緊急事態対策［424、192(45.3%)］、母子健康［239、155(64.9%)］、外傷［74、131(177.0%)］、職業病［51、102(200.0%)］、口腔衛生［39、71(182.1%)］と推定された。6段階評価による疫学および監視能力の評価では、2プログラム(テロおよび緊急事態対策、母子健康)にて改善、他の6プログラムでは低下していた。また、50州のうち31州(62.0%)が健康状態のモニターおよび健康に関する問題の解決、29州(58.0%)が診断および健康問題、危険に関する調査に対し、ほぼ完全な能力を有していると回答した。保健局で働く疫学者は修士課程修了者が最も多く(41.8%)、次いで博士課程(25.6%)、学士課程(23.2%)、準学士/高校卒業(5.0%)であった。疫学者の学位は1,897/2,580名(51.9%)が有し、369名(19.4%)はその他の疫学における専門教育を受け、541名(28.5%)は疫学に関し専門の教育は受けていなかった。また、疫学に関する能力を向上させるための教育の実施には、州および地域によって格差があり、公衆衛生診療の評価、疫学研究計画、指導および管理能力、統計ソフトを用いたデータ分析、調査システムの企画および評価、データの収集法などに関する教育が必要とされる。
