糖尿病関連予防的ケアの実施―グアム,2001～2003年

糖尿病患者における2010年国民健康目標は、予防的ケアとして年1回の眼と脚部の検査、最低1日1回の自己血糖値測定(SMBG)、最低年2回のHbA1c測定を実施することである。グアムの糖尿病患者における予防的ケアの実施状況を調査するため、2001―2003年のGuam Behavioral Risk Factor Surveillance Systemのデータを分析した。この期間中、グアム在住成人における糖尿病有病率は11%であった。年1回の眼検査実施率は65.6%(目標実施率75%)、脚部検査実施率は70.4%(目標実施率75%)、1日1回のSMBG実施率は32.2%(目標実施率:60%)、年2回以上のHbA1c測定実施率は56.7%(目標実施率:65%)であり、全ての実施率が2010年国民健康目標値よりも低かった。男性は女性よりもSMBG実施率が有意に低く(21.4%対56.7%)、18―44歳群は65歳以上群よりも年2回以上のHbA1c測定実施率が有意に低かった(42.5%対71.1%)。糖尿病患者の健康転帰悪化を防ぎ国民健康目標を達成するために、SMBGを推奨すると共にGuam Diabetes Prevention and Control Programの予防的ケアプログラムとサーベイランス活動を持続するべきである。
