Interventions to Increase Influenza Vaccination of Health-Care Workers - California and Minnesota

医療従事者のワクチン接種率増加のための介入－カリフォルニア,ミネソタ州

2003年における医療従事者(HCW)のインフルエンザワクチン接種率は40.1%と低く、患者への感染などの問題から接種率の増進のための戦略を紹介する。カリフォルニア州では2002年春からHCWにおけるインフルエンザワクチン接種率の調査を行っており、2002―2003年期には南部70ヶ所の養護施設(地域14%に相当)を対象に、A群:介入なし(25施設)、B群:教育的介入を実施(15施設)、C群:ワクチン接種日を設定(15施設)、D群:教育的介入および接種日を設定(15施設)に分け、それぞれの施設における接種率が調査された。67施設(95%)が調査に参加、7,123名中4,338名(61%、施設あたり56―68%)のHCWが接種後のアンケートに回答した。その結果、A群における接種率は27%であり、B群:34%、C群:45%、D群:53%と、ワクチン接種日設定による接種率の有意な増加が認められ、教育的介入による効果は認められなかった。ミネソタ州では1985年よりMobile Vaccination Cart Programが行われ、専門の看護師を乗せた車が各地域を巡回し、巡回予定を各医療機関に事前に連絡し、HCWは都合によりいずれの場所での接種が受けられるものとした。接種率は1996―1997年期:46%(1,475/3,177名)から漸増し、2003―2004年期には65%(1,950/3,008名)となった。また、ミネソタ州ローチェスターのMayo Clinicでは1999―2000年期の接種率が53.6%であり、積極的な接種奨励(接種を呼びかける広告や接種後の賞品配布など)により高い接種率を維持し、2003―2004年期には26,261名のHCWにおける接種率は76.5%であった。
