Legionnaires Disease Associated with Potable Water in a Hotel - Ocean City, Maryland, October 2003-February 2004

ホテルの飲料水に関連したレジオネラ症－メリーランド州,オーシャンシティー,2003年10月～2004年2月

2003年10月―2004年2月、メリーランド州オーシャンシティーのあるホテル(Aホテル)の宿泊客においてレジオネラ症(LD)8例が確認された。この報告では、その後Worcester County Health Department(WCHD)、Maryland Department of Health and Mental Hygiene(DHMH)、CDCにより実施された調査結果の概要を紹介する。2003年12月1日、地域健康局よりDHMHにメリーランド州住民のLD2例が報告された。この2例は2―10日間の潜伏期間中にAホテルにそれぞれ3、4日間宿泊し、ホテル出発から8、5日後に発症した。2例ともX線学的に肺炎と確認され、Legionella尿抗原検査陽性でL.pneumophila血清型1(Lp1)感染と一致した。これら2例の確認後広範なサーベイランスが実施され、新たに2003年10月―2004年2月にAホテルに宿泊した6例がLDと確認された(1例は可能性例)。全8例(37―70歳)のAホテルでの宿泊日数は1―4泊(中央値:3泊)で、ホテル出発から4―9日後(中央値:7.5日)に発症していた。また全確認例は部屋でシャワーを浴びるか入浴をしており、プールやスパを利用した者もいた。2003年12月―2004年2月、WCHD、DHMH、CDCはAホテルの環境調査と水質検査を行った。その結果、ホテルの様々な場所から採取した多くのサンプルでLp1が検出され、感染源はホテルの飲料用温水システムである可能性が高かった。2004年1月26日、Aホテルは改善策として給水システムの過熱処理、全ての水道蛇口の洗浄、冷却塔の高塩素処理を行った。また患者が宿泊した部屋のシャワーと蛇口を消毒し、シャワーヘッドと洗面台の通気装置を取り替えた。旅行関連LDのタイムリーな発見と感染コントロール実施のためには、広範な州ベースのサーベイランスが重要である。
