Tuberculosis Transmission in a Homeless Shelter Population - New York, 2000-2003

アメリカでは結核症例は年々減少しており、2003年には人口10万人あたり5.1であったが、ホームレスでの発症率は高く(2003年:6.8%)、今回、2003年1月―7月にかけてニューヨークOrange County Health Department(OCHD)にホームレスの結核症例が4例報告され、それ以前には2001年にNew York State Department of healthで2例、2002年にはNYC Department of Health and Mental Hygieneに2例の結核症例が報告されていた。これを受け2003年9月13―15日、ホームレスの収容施設居住者を対象にスクリーニング検査が実施された。1,083名中958名(92%)が参加し、927名がレントゲン検査、593名がツベルクリン検査を受け、判定を受けた583名中223名(38%)が陽性と判定され、スクリーニング前に陽性と判定されていた213名と合わせ、46%がツベルクリン陽性であった。さらに51例に対し喀痰培養検査を行い、4例が結核症例と確診された。また施設の従業員を対象とした検査では、16例がツベルクリン陽性であったが、確診例はなかった。2000―2003年における結核確診例は計29例であり、年齢49(39―81)歳、全例黒人、13例(45%)が国外にて出生していた。12例(41%)は痰標本が抗酸菌陽性を示し、7例はレントゲン検査にて空洞病巣を呈した。26例の遺伝子型分析では、11例にM. tuberculosisを認め(施設関連菌)、症例の平均居住期間は2.5年、食事や娯楽は共用スペースであった。他の遺伝子型はC-strain(NYCにて2001―2003年に検出)を8例にて認め、7例は非関連株、3例は不明であった。
