Tuberculosis Transmission in a Renal Dialysis Center - Nevada, 2003

腎臓透析センターにおける結核感染－ネバダ州,2003年

2003年、ネバダ州にある外来腎臓透析センターの透析技師が肺結核を発症した。この症例は診断前から6週間にわたる咳と疲労感を自覚しており、体重も14パウンド(約6.3kg)減少していた。喀痰顕微鏡検査では抗酸菌(AFB)陽性(4+)を示し、培養検査にてM. tuberculosisの存在が認められた。感染は診断3ヶ月前と判断され、その間に接触のあった患者や職員481例のうち、結核コントロールプログラム管轄外の症例48例を除いた433例(透析患者291例、職員92例、その他47例)を対象にツベルクリン(TST)が行われた。評価例333例(77%)のうち36例(11%)はTST陽性歴を有しており、残り297例のうち48例［16%:職員13例(19%)、患者29例(15%)］が陽性を示した。感染した職員と、非感染例の年齢、接触時間、人種/民族に有意差は認めなかったが、患者では非感染例に比べ高齢であった［感染例65歳(44―88歳)、非感染例60歳(11―95歳)］。感染例には週2回9ヶ月間のイソニアジド投与が行われ、9月10日の時点で12例が治療終了、2例が他の原因により死亡したが、二次感染例は認めていない。
