WHOとNational Respiratory and Enteric Virus Surveillance System(NREVSS)の協同研究所、州および地域保険局、医療提供者、人口統計登録所、CDCにより収集された情報に基づき、アメリカおよび世界における2003―04年インフルエンザシーズン中のインフルエンザ活性と、2004―05年シーズン用のインフルエンザワクチンの組成を紹介する。アメリカでは、2003―04年インフルエンザシーズンは例年より早く始まり、12月にピークを迎えた。最も多く確認されたのはインフルエンザA(H3N2)であり、インフルエンザBとインフルエンザA(H1)も少数確認された。肺炎およびインフルエンザ関連死亡率は連続9週間(12月20日―2月14日)流行閾値を超え、2004年1月10―17日の週に最高(10.3%)となった(過去4回のインフルエンザシーズン中の最高死亡率:8.1―11.2%)。世界各地でもこのシーズン中にはインフルエンザA(H1)、A(H3N2)、Bウイルスが循環し、インフルエンザA(H3N2)ウイルスが優勢であった。ベトナムとタイで報告された家禽類における鳥インフルエンザA(H5N1)の感染確診例は、重度呼吸器疾患や死亡と関連した。最近確認されたインフルエンザウイルスの抗原分析、疫学的データ、ヒトでのワクチン接種後の血清学的試験に基づき、FDAのVaccines and Related Biological Products Advisory Committee(VRBPAC)はアメリカにおける2004―05年の3価インフルエンザワクチンにA/New Caledonia/20/99様(H1N1)、A/Fujian/411/2002様(H3N2)、B/Shanghai/361/2002様ウイルスを含めるよう勧告した。
