Voluntary HIV Testing as Part of Routine Medical Care - Massachusetts, 2002

2003年、CDCはAdvancing HIV Prevention-New Strategies for a Changing Epidemicを発表した。このイニシアチブにおける4つの戦略のうち1つは、ルーチンでの任意のHIV検査を拡大することである。この報告では、病院関連緊急ケアセンター4施設の入院患者に対しHIVに関するカウンセリング(Counseling)・検査(Test)・専門医の紹介(Referral)を行うマサチューセッツ州のプログラム(HIV CTR)の成果を紹介する。このプログラムが開始された2002年には、4施設で全入院患者の約10―15%にあたる10,352例がHIVのカウンセリングを勧められた。この10,352例のうち7,071例(68%)はHIVのリスクを感じていない、以前HIV検査を受けたことがあるなどの理由により検査を拒否した。検査を受けた3,068例中60例(2.0%)はHIV感染者であった。データが入手できた42例全例が、経過観察のためにその後少なくとも1回はHIVケアを受けていた。また感染判明後に医師の診察やケアを受けていなかった6例に対しては、経過観察のためHIVケアを受けるよう指示した。このプログラムの資金は、Massachusetts Department of Public Health(MDPA)AIDS Bureauにより提供されている。このプログラムの最初の1年間の全コストは、349,400ドルであった。以上の結果は、ルーチンでのHIV CTRがHIV患者同定に有効であることを強調している。
