Invasive Streptococcus pyogenes After Allograft Implantation - Colorado, 2003

同種組織片移植後の侵襲性Streptococcus pyogenes－コロラド,2003年

同種組織片は様々な整形外科的処置(靱帯再建、半月板移植、脊髄手術など)で使用される。2003年9月、コロラド州の外来手術センターで17歳の男性が片側膝蓋腱同種組織片を用いた選択的前十字靱帯再建術を受けた。術後6日目、切開部位の疼痛、紅斑、発熱、悪寒を発症し、血液、創傷部の吸引検体、除去した移植組織の培養にてStreptococcus pyogenes［A群連鎖球菌(GAS)］の増殖が認められた。この患者で用いられた同種組織片は死亡ドナー由来であり、tissue processor A(TP-A)より供給されていた。同一ドナー由来腱の同種組織片は他に5例に移植されたが、12月1日現在、それら患者において予後不良例は確認されていない。診療記録によると、ドナーは死亡3週間前に頚椎固定術を受けており、剖検所見より死因は筋弛緩薬および鎮痛薬の毒性によるものとされていた。TP-Aは同種組織片に無菌処理と抗菌処理を行っていたが、滅菌処理は行っていなかった。遺伝子の塩基配列分析などの調査結果から、この患者におけるGASの伝播には汚染された組織片が関与していることが示唆された。同種移植感染症はまれであるが、今後移植組織の評価法と標準的処理法の改善が必要と考える。
