Detection of West Nile Virus in Blood Donations - United States, 2003

献血血液中の西ナイルウイルスの検出－アメリカ,2003年

2002年、アメリカでは西ナイルウイルス(WNV)血症ドナー16例由来の血液成分を輸血された23例がWNVに感染したと報告され、献血した可能性のあるウイルス血症ドナーは500例と推定された。アメリカでのWNV伝播再燃の可能性を考慮し、血液収集機関(BCA)は全献血者のスクリーニングと感染性血液成分の回収のためにWNV核酸増幅検査(NAT)を実施した。さらにFDAは、ドナーに献血1週間前に頭痛を伴った発熱がみられたか質問し、そのような症状がみられた場合には採血を延期するようBCAに勧告した。ここではWNV血症ドナーのNATによるWNVスクリーニング法と、6月下旬－8月上旬に献血した約95%の血液における検査結果を示す。スクリーニング法はロシュ社のリアルタイム定量RT-PCR(Taqman)法とジェンプローブ社-カイロン社のTMA(transcription-mediated amplification)法である。RT-PCR法は6検体、TMA法は16検体のサンプルを混合し、陽性であれば個別に検査する。なお、個別に検査を行っている機関もある。8月5日現在、スクリーニングを行った約100万件の献血のうち329件(約0.03%)がNATフォーマットにてWNV感染陽性であった。329件中163件(全献血者の約0.015%)はその後のNAT追加検査でも再度陽性を示した。8月5日現在、コロラド、フロリダ、ルイジアナ、ミシシッピ、ニューメキシコ、サウスダコタ、テキサス州の健康局はWNV血症ドナーと推定される38例を、CDCと57の州および地方健康局による共同サーベイランスプロジェクﾄであるArboNETに報告した。本スクリーニング検査はウイルス血症ドナーの特定と感染血液成分の使用防止に有効である。
