小児期早期死亡における特定の代謝病の関与－バージニア,1996～2001年

乳幼児突然死症候群(SIDS)あるいは1歳未満の乳児の原因不明の死亡はアメリカにおける乳児の死因の第3位である。1歳以上の小児でも原因不明の突然死が起こるが、その発生数は明らかにされていない。SIDSおよび原因不明の乳児突然死は未認識の代謝病の合併が関与している可能性がある。CDC、バージニア州検死局(ME)、民間研究所はその代謝病に関連した原因不明の小児期早期突然死の割合を検討するため地域住民をベースにした研究を実施した。1996－2001年のバージニア州MEの記録をレビューし、3歳未満で死亡した904例のうち、死後の代謝スクリーニング結果と死後保存血のいずれも入手できなかった症例を除く793例(88%)について分析した。793例中8例(死亡時年齢中央値7.5ヶ月、生後2日－2.7歳)がタンデム質量分析法(tandem MS)によるスクリーニングにて代謝病を示唆する陽性結果を示した。8例中4例では脂肪酸酸化異常を示唆するスクリーニング結果が得られ、4例では有機酸血症を呈している可能性があった。性別、人種/民族、年齢とスクリーニング陽性結果との関連はみられなかった。これら8例中7例は、新生児期に疾患が確認され治療を受けて改善した可能性があった。新生児スクリーニングプログラムにおけるtandem MSを使用した代謝病検査により、小児期早期死亡を未然に防げる可能性が考えられた。
