6ヶ月齢－12歳の小児を対象とした全国麻疹予防接種運動－アフガニスタン,2002年

アフガニスタンでは23年間に及ぶ内戦のため公衆衛生の基盤が壊滅状態にあり、乳児および5歳以下の小児の死亡率が世界で最も高い(生産児1,000人あたりそれぞれ165人、256人)。その主な死因は10歳未満の小児では下痢(32%)、麻疹(25%)、呼吸器感染症(13%)、その他(栄養失調、壊血病、慢性疾患、原因不明の発熱など、30%)であり、麻疹による死亡者数は毎年30,000－35,000人に及ぶ。1998－2001年における予防接種率は低く(乳児で40－47%)、2001年の乳児における3種混合[ジフテリア、破傷風、百日咳(DPT)]接種率は45%、ポリオ46%、麻疹47%であった。2001年、麻疹症例は8,762例報告され、うち8,356例(95%)が12歳以下の小児であった(1歳未満:8%、1－5歳:57%、5－12歳:35%)。2001年後半の時点で、2002年における周辺諸国からの難民流入による麻疹の流行が予想されたため、麻疹による死亡者の減少を目的に、アフガニスタン暫定政府の厚生省、WHO、UNICEFは6ヶ月齢－12歳児を対象とした全国的な予防接種運動を実施した。国内機関やNGOにより1,200ヶ所の接種施設および3,000台の移動診療所が提供され、ハイリスク地域や対象児数の多い都市から接種を開始した。また難民にはイランやパキスタンとの国境で接種を行った。2002年12月31日現在、計10,299,878名(接種対象児の82%)に接種され、2001－2002年の麻疹症例は8,762例から2,574例に減少した。費用は小児1人あたり約$0.78であった。予防接種運動中にワクチン接種による死亡例は報告されなかったが、副作用調査システムが確立していないため、副作用の発現状況は明らかではない。
